I've had my EZ Pass for while now, initially because of the annoying $1 tolls going thru Chicago and because I pulled a toll ticket for an 18 wheeler in Indiana once and almost got smooshed crossing the many lanes over to the office to try to get it fixed.

I've had mine mounted to the windscreen, in my pocket and on this return from Rhode Island actually held it in my hand in the air as I went through the tolls in Mass and NY.

In NY I typically get "Call EZ Pass" or "No EZ Pass". Occasionally I'll get a green light, OK more like rarely.
Last Thursday I went to an EZ Pass center to have the pass checked, it is fine they said, along with "They don't work well on bikes"

What are your experiences and how do you mount/use your EZ Pass?

I'm about to give it up and just take a long time at each toll in order to share my frustation with the general public. OK maybe not...

Mine worked well 90%+ of yhe time.

The one place it did not like was Chicago.  
IN, OH, PA, MD, VA DE, NY, MA all ok.

Mount it at the correct angle - that seems to help.

I have had no issue with mine, but somethign you can do to eliminate problems is to go to the EZ pass website for your state and make sure your license plate # is in the system.  When you go through an EZ pass lane and your transponder does not work, a picture of you plate is taken. If you are in teh system you get billed the toll like normal.

I had mine mounted to the inside of the fairing.  It worked fine, and was out of sight, so nobody would walk off with it.

All the bikes are registered and EZ Pass has no problems with me. I should be happy that I don't trip the lights. I guess... Maybe I'll just stop worrying about it until EZ Pass has a problem with it.

I've never had a problem with mine, but I also have it mounted in a RAM mount on my handlebar. It's unubstucted and at about a 45 degree angle. It's been used up and down the east coast and also the Chicago/Indiana area. I've used it in all the areas you've mentioned.

I can't ever remember it not being picked up.

Never had an issue with mine.  PA actually has a motorcycle specific EZ pass token.  Can't use a car one as they won't let you add a bike to it. Can't use the bike one in the car.  They are both tied to the same account so at least you don't have to move the token from vehicle to vehicle.

I ride 1 inch off the center line in the high speed EZ pass lane and NEVER get charged. Don't carry the pass but have all the bikes plates registered with them. Don't tell too many about this.
